Terms and Conditions for Beneficiaries Accessing Free Services Under PM-JAY and CMHIS
By availing of free healthcare services under the PM-JAY and CMHIS scheme, beneficiaries agree to the following terms and conditions. Please read them carefully before proceeding.
1. Acceptance of Terms
By utilizing the benefits/services under PM-JAY and CMHIS, beneficiaries acknowledge and accept these terms and conditions.
2. Eligibility
This service is available exclusively to registered beneficiaries who meet the eligibility criteria established by PM-JAY and CMHIS, as well as individuals who are interested in and qualify for these schemes.
2. Participation in Feedback Surveys and Grievance Redressal
2.1 To enhance the quality of services and ensure effective grievance redressal, beneficiaries who access services under PM-JAY and CMHIS may be contacted for:

3.1 PM-JAY and CMHIS is committed to protecting the personal information of beneficiaries. Any data collected during feedback surveys or grievance redressal will be handled in accordance with applicable laws and the PM-JAY and CMHIS Privacy Policy.

4.3 Beneficiaries are advised to avoid sharing sensitive personal information unless necessary and to ensure that they are speaking with authorized personnel.
4.4 PM-JAY and CMHIS reserves the right to restrict a beneficiary's access to the free services if there is suspected misuse, violation of these terms, or other inappropriate conduct.
5. Limitation of Liability
5.1 PM-JAY and CMHIS shall not be held liable for any loss or inconvenience arising from a beneficiary's refusal to participate in feedback surveys or grievance redressal processes.
5.2 PM-JAY and CMHIS disclaims liability for any issues resulting from technical failures, third-party interference, or other events beyond reasonable control.
6. Modification of Terms
PM-JAY and CMHIS reserves the right to amend or modify these terms and conditions at its sole discretion without prior notice. Beneficiaries will be notified of any significant changes through notice on website.
7. Governing Law and Jurisdiction
These terms and conditions are governed by the laws of India. Any disputes or claims arising from these terms shall be subject to the exclusive jurisdiction of the courts in Kohima, Nagaland.
