Registration is open for all Categories of applicants, with the following criteria:-

1. "Proof of Relationship" document is required for all Dependants.
2. General Category Applicants , they have to upload the System Generated Self Declaration Form duly signed by both the applicant and concerned Village/Ward/Colony Chairman.
3. Government Employees
   1. Have to Provide their PIMS Code in the Registration Form
   2. Full Name will be as per PIMS
   3. Incase of Mismatch of Name between PIMS & AADHAAR, kindly UPDATE AADHAAR
   4. After adding all their Dependants, They have to upload the System Generated Self Declaration form duly signed by both the employee and concerned Head of Office.
4. Pensioners
   1. After Registration they have to Provide their PPO Code in the Application Form
   2. After adding all their Dependants, They have to upload the System Generated Self Declaration form duly signed by the pensioner.
   3. Also upload the PPO Book (first 3 pages) clearly indicating the Nominee/Nominee's
