Registration is open for all Categories of applicants, with the following criteria:

  • "Proof of Relationship" document is required for all Dependants except SPOUSE.
  • "Mobile Number" will be verified using OTP for login and registration.
  • "Email" is required for PASSWORD RESET & MOBILE NUMBER change. Email will also be verified.
  • "Email" of any Family Member can be used to register on the CMHIS Portal.

Government Employees

  • A valid PIMS Code is required
  • Full Name, DOB and Entitlement will be as per PIMS
  • Incase of Mismatch of Name between PIMS & AADHAAR, kindly UPDATE PIMS

Pensioners

  • PPO Number & PPO Book is mandatory.
  • After adding all their Dependants, first 3 (THREE) Pages of the PPO Book is required to be uploaded as PDF.
  • The Uploaded PPO Book should clearly indicate the Nominee/Nominee's Name
Register as Pensioner

General Category Applicants:

  • Indigenous Inhabitant Certificate or PRC is mandatory.
  • Application will be processed by the District Administration of IIC/PRC issuing district.
  • The IIC / PRC should be issued after 2016
  • Upload the System Generated Self Declaration Form duly signed by both the applicant and concerned Village/Ward/Colony Chairman.
