Registration Process:

Households can either self register in the portal or visit any of the registration centres for assistance in registering in the portal.

Valid email and mobile number will be required for registering.

  Mandatory KYC Document Required:

  a. For the Head of Household-Idigenous Inhabitant Certificate (IIC) or Permanent Residential Certificate (PRC) issued by competent authority of respective district Administration after 2016.

b. Filled and downloaded online registration form with household member details, countersigned by Village council Chairman/Ward Chairman. 

Approval will be from the District Administration from where IIC/PRC was issued.

After Registration

⦁ On approval, the applicant will get a confirmation message over his registered mobile number and email along with Household ID assigned for his family.

 

Γªü On receiving the Household ID, please visit any of the registration centre with aadhaar Card for Aadhaar eKYC and Ayushman CMHIS  Card  Issuance. 

 

Beneficiary Eligibility Criteria: 

All indigenous and/or Permanent residents of the State who are not beneficiaries under the AB PM-JAY or any public funded insurance scheme of the government and are not State government employees and pensioners.

RationCard Holders do not need to register again as a new household, and can get Ayushman CMHIS Card issued against their existing valid ration cards.

Household Definition shall be as per SECC definition of household as follows:

“A household is usually a group of persons who normally live together and take their meals from a common kitchen/common cooking unless the exigencies of work prevent any of them from doing so."
