1. Register with PPO number, email and mobile number (preferably mobile number linked with Aadhaar)
2. Enter Pensioner and spouse details, submit.
3. Add dependants with relevant supporting documents.
Note:
i.If both spouses are pensioners, the same must be declared in the application form, and will be considered together as a single family.
ii. Dependants for Pensioners shall be restricted to the nominees declared in the PPO book.
Approval will be from the Finance Department
After Registration
⦁ On approval, the applicant will get a confirmation message over his registered mobile number and email along with Household ID assigned for his family.
⦁ On receiving the Household ID, please visit any of the registration centre with aadhaar Card for Aadhaar eKYC and Ayushman CMHIS Card Issuance.
Beneficiary Eligibility Criteria:
State government Pensioners includes State government pensioners who are eligible for monthly Medical Allowance (MA) viz: (1). pensioners, (2). ex-legislators and (3). family pensioners, and their dependent family members.
2. Family Definition:
The term ‘family’ means and includes- “Husband or wife as the case may be and dependant family members.”
4. KYC Document Required:
a. Supporting documents for dependents while registering:
First Three Pages of PPO Book.
Note:
a. Families where both spouses are government employees/ pensioners/ Legislator/ex-Legislator, shall be considered as a single household for the purpose of Insurance Coverage, ward entitlement for the merged household shall be according to the higher entitlement of any of the spouses.
b. For this, the spouse with higher entitlement should register in the State Beneficiary portal as head of household, and declare the spouse under his/her household.
