eCard Issuance Process for different categories of beneficiaries will be as follows:

AB PM-JAY Beneficiaries

Please log on to mera.pmjay.gov.in to check eligibility under AB PM-JAY.

If your name features under the AB PM-JAY Beneficiary eligibility list, please follow the process to get an eCard in section 6.5.

 

BoCW Beneficiaries 

If you possess a valid BoCW registration card issued by the NBOCWWB,  follow the process to get an eCard in section 6.5, along with a copy of the BOCW registration Card.

 

Valid ration Card Holder  issued in the state

follow the process to get an eCard in section 6.5.

 

Uncovered population not featuring in any of the above categories(PM-JAY/BoCW /Ration Card)

There will be a registration process  to enrol process for those residents of the state that are not in any of the above listed databases.

 

Household Registration :

(in the State Beneficiary Base: cmhis.nagaland.gov.in)

(Registration may be done either by the individuals or by Hospital PMAMs or any of the authorised agencies like the CSCs, Clairvoyance Tech, Zephyr Ltd, etc)

 For self registration:

⦁ Log on to cmhis.nagaland.gov in

⦁ Fill up the form 1 (Registration oh households under General Population).

⦁ Take a printout of the filled form.

⦁ sign at the place for signature

⦁ Countersign by authorised signatory from the district Administration.

⦁ Scan the signed form, upload with all other necessary supporting documents.

⦁ Submit

Alternately, one may take the assistance of Hospital PMAMs or any of the authorised agencies like the CSCs, Clairvoyance Tech, Zephyr Ltd, etc) for the registration.

NB: Registration alone does not imply eligibility unless the person obtains the eCard following verification. 

 

 KYC Documents required 

1. Scanned copy of filled downloaded which has the complete household members list , self signed and countersigned by competent authority .

2. Permanent Residency Certificate(PRC)/Indigenous Inhabitant Certificate(IIC)

Registration Approval:

Household registration shall be approved by the District Administration of the district from which the PRC/IIC was issued. On approval by the District Administration, the applicant will get a confirmation message over his registered mobile number along with Household ID assigned for his family.

On receiving the Household ID, please follow the process in section 6.5 to get  the Ayushman CMHIS  Card issued for each and every approved household members 

Issuance of Ayushman CMHIS Card

Aadhaar eKYC is mandatory for issuance of eCard.

⦁ Walk-in to any public PHC/ CHC/ DH empanelled under AB PM-JAY in the state.

⦁ Common Service Centre or UTI-ITSL kiosk.

⦁ During the Outreach Enrolment Camps of AB PM-JAY CMHIS, eSHRAM or registration camps of various welfare schemes in your locality.
